Telehealth Informed Consent
Last updated: October 7, 2026
This Telehealth Informed Consent (“Consent”) explains what telehealth is, how it is used through the OneClick Rx platform, and the benefits, risks and limitations of receiving care this way. Please read it carefully. By completing a medical intake or requesting a consultation through oneclickrx.com, you confirm that you have read and understood this Consent and agree to receive care via telehealth.
OneClick Rx LLC (“OneClick Rx,” “we,” “us”) operates a technology platform and does not itself provide medical services. Medical services are provided by independent, state-licensed physicians, nurse practitioners and physician assistants (“Providers”) in our affiliated provider network. Prescriptions, if issued, are filled by independent, state-licensed pharmacies.
1. What Telehealth Is
Telehealth is the delivery of health care services using electronic communications, such as secure online questionnaires, messaging, photos, telephone and video, when you and your Provider are not in the same physical location. Depending on your state’s laws and your treatment, your consultation may be asynchronous (your Provider reviews the information you submit and responds through secure messaging) or synchronous (a live video or telephone visit).
2. Your Provider and the Provider–Patient Relationship
- A Provider licensed in the state where you are physically located at the time of your consultation will review your information.
- A provider–patient relationship is established only when a Provider reviews your information and agrees to treat you. Completing an intake or paying for a consultation does not by itself create that relationship.
- Your Provider uses independent professional judgment. Your Provider may decide that telehealth is not appropriate for you, may request more information, lab tests or a video visit, or may refer you for in-person care.
- There is no guarantee that you will be treated or that a prescription will be issued. Any prescription is issued only if your Provider determines it is medically appropriate.
- Telehealth through OneClick Rx is not a replacement for your primary care provider. We encourage you to keep a regular in-person health care provider and to share your treatment with them.
3. Possible Benefits
- More convenient access to care without travelling to an office.
- Faster access to a licensed Provider for evaluation and follow-up.
- Discreet care for sensitive health concerns.
4. Possible Risks and Limitations
- Your Provider cannot perform a physical examination. Information you provide may be incomplete, and some conditions may be missed or require in-person evaluation.
- Decisions are based on the information you submit. If it is inaccurate or incomplete, your treatment may not be appropriate and you could be harmed.
- Technical problems, such as poor connections, equipment failures or images that are not clear, may delay or interrupt your care.
- Although we use security safeguards, there is a small risk that electronic information could be intercepted or accessed without authorization.
- Medications prescribed through telehealth carry the same risks and side effects as medications prescribed in person. Your Provider will review the main risks with you, and you should read all information provided with your medication.
5. Not for Emergencies
Telehealth through OneClick Rx is not for medical emergencies or urgent conditions. If you think you are having a medical emergency, call 911 or go to the nearest emergency room right away. Messages sent through the platform may not be read immediately.
6. Your Responsibilities
By consenting to telehealth, you agree to:
- Be at least 18 years old and physically located in a state we serve at the time of each consultation.
- Provide complete, accurate and truthful information about your identity, medical history, current medications, allergies and symptoms, and update it if anything changes.
- Follow your Provider’s instructions, take medications only as prescribed, and not share your medication with anyone else.
- Report side effects or changes in your health to your care team, and seek in-person or emergency care when appropriate.
- Complete any lab tests, follow-up visits or check-ins your Provider requests.
7. Privacy and Medical Records
Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law, as described in our Notice of Privacy Practices and Privacy Policy. Your Provider will keep a medical record of your telehealth consultations, and you have the right to request a copy. Your information may be shared with the pharmacy that fills your prescription, with laboratories, and with others involved in your care or payment, as permitted by law.
8. Electronic Communications
You agree that we and your Provider may communicate with you electronically, including through the secure patient portal, email and text message (SMS), about your care, prescriptions, shipments and account. Standard email and text messages may not be fully secure; do not use them to send detailed health information. You may update your communication preferences at any time by contacting support@oneclickrx.com.
9. Prescriptions and Pharmacy
If your Provider prescribes medication, it will be sent electronically to a state-licensed partner pharmacy that ships to your address, unless you ask your Provider to send it to a pharmacy of your choice where available. Some medications, including compounded medications, are not FDA-approved; your Provider will explain your options. All prescribing is done in compliance with applicable federal and state law.
10. Payment
Telehealth services through OneClick Rx are self-pay and are not billed to Medicare, Medicaid or other insurance. Fees are shown before you check out. Please see our Returns & Refund Policy and Terms of Use for details.
11. Your Rights
- You may ask questions about telehealth, your Provider’s credentials and your treatment at any time.
- You may decline telehealth or withdraw this Consent at any time by contacting support@oneclickrx.com. Withdrawing consent will not affect care already provided.
- You may request in-person care instead of telehealth. We can help you find options, but OneClick Rx does not offer in-person visits.
12. State-Specific Requirements
Some states have additional telehealth rules, such as requiring a live video visit before certain medications are prescribed. Your Provider will follow the requirements of the state where you are located, which may change how your consultation is conducted.
13. Acknowledgment
By checking the consent box during intake or by continuing with a consultation, you acknowledge that you have read this Telehealth Informed Consent, that you understand the benefits, risks and limitations of telehealth, that your questions have been answered or that you know how to ask them, and that you voluntarily consent to receive health care services via telehealth.
